Time is often of the essence when it comes to infectious disease, nephrology (which involves the kidneys), endocrinology (treating the endocrine glands and hormones), and palliative care medicine (which aims to relieve the symptoms of serious illnesses). For example, says Dr. Siegan, "When a patient arrives at the hospital in a diabetic crisis, the admitting physician will likely want to consult with an endocrinologist. If the endocrinologist is across town at a different hospital, they can use the telemedicine carts to examine the patient and immediately develop a treatment plan." A clinical expert operates each cart, wheeling it into a hospital room, where the doctor appears on a monitor, able to see and speak with the patient. The specialist gathers necessary vitals and information by way of the cart's high-definition camera and Bluetooth-equipped medical instruments— such as a stethoscope that allows the physician to remotely examine the heart and lungs or an otoscope that is used to examine the eyes—manned by the clinical expert. "Patients will always have a hospitalist or bedside physician on site captaining their care team," stresses Dr. Siegan, adding that the specialist will follow up in person, if needed. "Telemedicine is simply a support service—a way to get doctors to their patients immediately when that otherwise wouldn't be possible." And ultimately, that's what the latest digital innovations are all about: delivering the right care to patients right when they need it. Dr. Mitchell Siegan HOW TO ENROLL Read on for how to use Roper St. Francis Virtual Care: ➊ Visit from a computer, or download the free Roper St. Francis Virtual Care app from the App Store or Google Play for your smartphone or tablet. ➋ Fill in basic information about yourself and your health history. You will need a valid photo ID (a driver's license, military identification, passport, or state-issued identification card). ➌ Pay the $59 fee using credit, debit, or HSA card. Some employers and health insurance plans cover a virtual care visit—call your human resources department or insurance company to find out if this applies to you. ➍ Choose from a list of healthcare providers. You'll see their photo, name, oce location, and whether any patients are in their virtual waiting room. ➎ The selected provider will review your chart and health concerns then begin the video consultation. The visit will go much as it would in person, with the provider asking questions about your condition and asking you to assist in gathering any physical evidence needed (you may need to demonstrate your breathing, for example). ➏ The provider makes a diagnosis and provides a treatment plan. If they determine that you should be seen in person, they'll navigate you to the appropriate provider. If a prescription is needed, it will be sent to the pharmacy of your choice. (No narcotics or controlled substances can be prescribed.) ➐ You'll receive a summary of your visit, and records can also be shared with your primary care provider.
